1. Acceptance of Terms

By ordering equipment, receiving delivery, or browsing reliabledmepro.com, you agree to be bound by these Terms and Conditions and all applicable laws and regulations governing Durable Medical Equipment, Prosthetics, Orthotics, and Supplies (DMEPOS).

2. Assignment of Benefits & Insurance Billing

When you request that Reliable Medical Equipment bill Medicare, Illinois Medicaid, Medicare Advantage, or commercial insurance on your behalf:

• You authorize Reliable Medical Equipment Supply Inc. to submit claims and collect payments directly from your insurer.
• You acknowledge that verification of benefits does not guarantee claim payment. If your insurance plan denies coverage or requires a copay, coinsurance, or unmet deductible, you remain financially responsible for the balance.
• You agree to supply current insurance cards, physician orders, and medical documentation required for prior authorization.

3. Rental Equipment Terms

For items provided on a rental basis (such as hospital beds, manual wheelchairs, or patient lifts):

• Title and Ownership: Equipment remains the property of Reliable DME unless ownership is transferred under Medicare 13-month capped rental rules or purchase agreement.
• Maintenance and Care: The patient or caregiver agrees to use equipment only as instructed and keep it in a safe, clean environment. Normal wear and tear is maintained by Reliable DME at no additional cost.
• Notification of Pickup: The customer must notify Reliable DME immediately when equipment is no longer medically necessary so pickup can be scheduled and recurring billing terminated.

4. Manufacturer Warranties

Reliable Medical Equipment honors all manufacturer warranties on new equipment sold, in accordance with Medicare DMEPOS Supplier Standard #6. We assist customers with warranty repairs or manufacturer replacements where applicable.
